TABLE 2 (CONTINUED) <br />HAZARD ANALYSIS <br />EXPLANATIONS <br />Exposure: The frequency of exposure to the hazard event. <br />A = Continuously <br />B = Frequently <br />C = Occasionally <br />D = Seldom <br />Probability: The likelihood that and injury will occur upon exposure to <br />the hazard event. <br />A = Certain or almost certain <br />B = Likely, not unusual, 50/50 chance of occurring <br />C = Unusual, would generally occur less than 50% of the time <br />D = Improbable, very low chance of occurrence <br />Consequence: The degree of injury resulting from exposure to the hazard <br />event of significant enough degree to cause an injury. <br />A = Fatality <br />B = Serious injury, including chemical exposure, requiring <br />hospitalization <br />C = Moderate injury, including chemical exposure, requiring on- <br />site first aid treatment <br />D = Minor injury, including chemical exposure, requiring on-site <br />first aid treatment <br />E = Chemical, acoustical, or other exposure above the Threshold <br />Limit Value (TLV) or other recommended standard that may not <br />produce immediate acute effects, especially for chronic <br />toxicants <br />
