Integrity testing has not been conducted on the aboveground petroleum containers. Integrity testing <br /> is not addressed in the SPCC Plan. The facility shall test or inspect each aboveground container for <br /> integrity on a regular schedule and whenever you make material repairs. Examples of these integrity <br /> tests include, but are not limited to: visual inspection, hydrostatic testing, radiographic testing, ultrasonic <br /> testing, acoustic emissions testing, or other systems of non-destructive testing. Comparison records <br /> and other records of inspections and tests must be maintained on site with the SPCC Plan. Immediately <br /> conduct all necessary integrity testing and maintain records on site as required, and submit copies to the <br /> EHD. <br /> Complete and submit a copy of the Return to Compliance Certification to the EHD by July 7, 2011 along <br /> with a statement documenting the corrective actions that have been or will be taken for each violation <br /> and any supporting paperwork. <br /> Note: The facility SPCC Plan does not include procedures for written testing and inspections. Monthly <br /> inspections are being conducted on the aboveground tanks during the routine underground tank <br /> inspections. <br /> Note: According to Mr. Fouts, the diesel tanks were added to the facility inventory in February 2011. <br /> The SPCC Plan must be amended when there is a change in the facility design, construction, operation, <br /> or maintenance that materially affects its potential for a discharge, within 6 months of the change, and <br /> implemented as soon as possible, not later than 6 months following preparation of the amendment. <br /> Provided: Return to Compliance Certification form <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By' <br /> EHD 23-02 03 ev 08/10/10 CONTINUATION FORM <br />
