SPCC Plan failed to include correct amount of product type and storage capacity. In SPCC Plan, it <br /> stated on page 11, Hot plant includes 4 ASTs (one 1 OK gallon diesel, two 30K gallon hot asphalt oil, one <br /> 1 OK gallon hot asphalt oil). The one 1 OK gallon diesel has been removed as documented in the <br /> December 2008 Monthly Tank Inspection Checklist and Documentation form by Brian Hoffman. Correct <br /> by revising the correct product types and storage capacity listed on facility. <br /> - Corrected onsite. Envir Crd Ms. Tobia revised it to 3 ASTs. <br /> #24. SPCC Plan failed to include the correct phone number of the local agencies. In the SPCC Plan, <br /> section Emergency Notification, it states an incorrect phone number for SJC Office of Emergency <br /> Services. Contact list and phone numbers for facility response coordinator, NRC, cleanup contractors <br /> with whom you have an agreement for response, and all appropriate Federal, State, and local agencies <br /> who must be contacted in case of discharge. Correct by including the updated phone numbers for OES. <br /> - Corrected onsite. Envir Crd Ms. Tobia revised the SJC OES phone number. <br /> #46. Failed to provide sufficient secondary containment capacity for bulk storage tank that would hold <br /> the volume of the largest container plus sufficient freeboard to contain precipitation. In the fuel <br /> containment area, there is a cement berm around the assortment of petroleum bulk tanks but at the <br /> bottom of the containment are 3 rectangular cutouts. Adjacent to the fuel containment area is the vehicle <br /> loading which the cement floor is sloped at east and west sides, south side with a wall berm. The north <br /> side of the vehicle loading borders the fuel containment berm to allow spillage to enter through the 3 <br /> rectangular cutouts into the fuel containment area. Secondary containment for the assortment of <br /> petroleum bulk tanks is breached and is not sufficient to contain precipitation. Correct by submitting a <br /> statement how the fuel containment area and the vehicle loading area will be provided with sufficient <br /> secondary containment. <br /> # 50. Failed to regularly test approved liquid level sensing devices. The two 20K gallon diesel tanks <br /> have an liquid level sensing device (automatic tank gauge) that monitors liquid level for inventory <br /> purposes. Per Ms. Tobia, there is an audible sound that alerts if tank overfills. It is unknown where the <br /> audible sound is located. It is not documented in the SPCC plan about the overfill mechanism and the <br /> time scheduling for testing the liquid level sensing device. Correct by submitting a statement how the two <br /> 20K gallon diesel tanks will be tested for overfill prevention and include the method in the Plan. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILLPE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTI,94 AT ANY TIME AT H 'S CURRENT HOURLY RATE. <br /> EH71eror: Re,Ceived By: Ti <br /> / G <br /> EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
