Hwy 99, Stockton Program: APSA <br /> SUMMARY OF VIOLATIONS <br /> CLASS I, CLASS II, or MINOR-Notice to Comply) <br /> **The following issues were noted based on the review of the SPCC Plan (submitted in response to the <br /> March 23, 2011 inspection) conducted on May 10, 2011.** <br /> --The SPCC Plan was last reviewed on November 1, 2010. A review and evaluation of the plan must be <br /> conducted at least once every 5 years. You must document your completion of the review and <br /> evaluation, and must sign a statement as to whether you will amend the Plan, either at the beginning or <br /> end of the Plan, or in a log or appendix to the Plan. The following words will suffice, "I have completed <br /> the review and evaluation of the SPCC Plan for (name of facility) on (date), and will (will not) amend the <br /> Plan as a result." Immediately amend the SPCC Plan to include all necessary documentation of the Plan <br /> reviews and ensure that future Plan reviews are properly documented. <br /> --The management approval portion of the SPCC Plan has not been signed. The Plan must have the <br /> full approval of management at a level of authority to commit the necessary resources to fully implement <br /> the plan. Immediately obtain required management approval for the SPCC Plan for this facility. <br /> --The underground storage tank included in the facility diagram is not listed as "exempt." The Plan shall <br /> include a facility diagram which must mark the location and contents of each fixed storage container and <br /> the storage area where mobile or portable containers are located. It must identify the location of and <br /> mark as "exempt" underground tanks. It must also include all transfer stations and connecting pipes, <br /> including intra-facility gathering lines. Immediately update the facility diagram to include all of the <br /> required information. Submit a legible copy of the updated facility diagram to the EHD. <br /> --The monthly inspection form being used is not the form provided by the SPCC Plan, and in some <br /> instances, does not contain the same information. The forms submitted with the SPCC Plan are for <br /> required air pollution inspections. Inspections and tests must be conducted and stored in accordance <br /> with the written procedures developed for this facility in the SPCC Plan. Immediately begin conducting <br /> all inspections and tests in accordance with the procedures in the SPCC Plan, or amend the Plan and <br /> have it recertified by a Professional Engineer to accurately reflect the inspection procedures currently <br /> followed at the facility. <br /> --Annual training records were not submitted with the SPCC Plan. At a minimum, oil-handling personnel <br /> must be trained in the operation and maintenance of equipment to prevent discharges, discharge <br /> procedure protocols, applicable pollution control laws, rules & regulations, general facility operations, <br /> and the contents of the facility's SPCC Plan. Annual briefings must be conducted to assure adequate <br /> understanding of the SPCC Plan and highlight and describe any known discharges or failures, <br /> malfunctioning components, and any recently developed precautionary measures. Immediately conduct <br /> necessary training for all oil handling personnel and submit copies of training records to the EHD. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> 11✓(Qa��G�� Citi <br /> EHD 23-02-0 ev 05/28/10 CONTINUATION FORM <br />
