According to Mr. Dan Souza, a 90-gallon 80/90 gear oil container is being utilized onsite and this cor _..ner is not addressed in the Spill Prevention, Control, and Countermeasure (SPCC) Plan. The following shall be addressed in the Spill Prevention, Control, and Countermeasure (SPCC) Plan: -type of oil in each fixed container and it's storage capacity. For mobile or potable containers, the type of oil and storage capacity for each container or an estimate of the potential number of mobile or portable containers, the types of oil, and anticipated storage capacities -discharge prevention measures including procedures for routine handling of products -discharge or drainage controls such as secondary containment, equipment, and procedures for the control of a discharge - countermeasures for discharge discovery, response, and cleanup - methods of disposal of recovered materials - contact list and phone numbers for the facility response coordinator, National Response Center, cleanup contractors, and all appropriate Federal, State, and local agencies. Immediately amend the SPCC Plan to include all required information. This is a repeat violation, Class II. 614 CFR 112.7(1)(1) Failed to train personnel on discharge prevention. No recent training records (2013, 2014, and 2015) were available for the oil handling personnel. At a minimum, oil handling personnel shall be trained in the operation and maintenance of equipment to prevent discharges; discharge procedure protocols; applicable pollution control laws, rules, and regulations; general facility operations; and the contents of the Spill Prevention, Control, and Countermeasure Plan. Immediately provide this training to all oil handling personnel and submit a copy of the training log to the EHD. This is a Class II violation. 616 CFR 112.7(f)(3) Failed to schedule and conduct discharge prevention briefings at least annually. No recent training records (2013, 2014, and 2015) were available for the oil handling personnel and as such the appropriate training could not be verified. Discharge prevention briefings are not scheduled at least once a year. Discharge prevention briefings for oil handling personnel must be scheduled and conducted at feast once a year to assure adequate understanding of the SPCC Plan for that facility. Such briefings must highlight and describe known discharges or failures, malfunctioning components, and any recently developed precautionary measures. Immediately schedule and conduct a discharge prevention briefing, ensure that they are scheduled and conducted at least once a year. This is a Class II violation.
