Cluff Avenue, Lodi and transport it to 4988 E. Highway 12, Lodi, <br /> CA, for the purpose of soil aeration. <br /> Prior to authorizing.the transport of contaminated soil from the underground storage tank(UST)excavation,. <br /> and:final closure.on the UST removal site, the following information shall be submitted to PHS-EHD: <br /> 1. Provide documentation that Frank C. Alegre is the legal owner of 4988 E. Highway 12, Lodi, CA. <br /> I <br /> 2. Provide documentation from the San Joaquin County Air Pollution Control District authorizing the F <br /> f proposed aeration of the diesel and gasoline contaminated soil at 4988 E. Highway 12, Lodi, CA. <br /> 3. Determine the amount(in cubic yards) of contaminated soil proposed for aeration at 802 N. Cluff <br /> Ave., Lodi, CA. <br /> 4. Determine if:there are workers or residences in close proximity to 4988 E Highway 12, Lodi, CA, <br /> where aeration of contaminated soil is being considered. <br /> 5. Provide the name, address, and the EPA number from the Department of Health Services for the <br /> Hazardous Waste Hauler contracted to transport the contaminated soil. <br /> 6. Provide the San Joaquin County Environmental Health Division - Local Oversight Program a <br /> workplan and associated fees to reexcavate and resample the-UST excavation at 802 N.
