Cluff Avenue Mlll <br /> Lodi, California 95240 COMPANY, INC <br /> Item#103: HSC 25270.6{a) Failed to file HMBP or annual facility tank statement. <br /> Summary of Violations: <br /> • A tank facility statement or business plan has not been submitted since 05/01/2017. <br /> • A tank facility statement identifying the name and address of the tank facility, a contact <br /> person for the tank facility,the total storage capacity of the tank facility, and the location, <br /> size, age, and contents of each storage tank that exceeds 10,000 gallons in capacity and that <br /> holds a substance containing at least 5 percent of petroleum shall be submitted annually. <br /> • Submittal of a business plan satisfies the requirement to submit a tank facility statement. <br /> • Immediately submit a tank facility statement or business plan. <br /> • This is a Class II violation. <br /> Corrective Actions To Be Taken For Each Violation: <br /> 1. The CERS profile (for Cluff facility) will be reviewed, updated, and submitted to CUPA via CERS. <br /> The review includes the following documents. <br /> a. Consolidated Emergency Response/Contingency Plan (attached) <br /> b. Ford Construction Yard Map with Grid (attached) <br /> c. List of Oil and Containers at the Facility (complete the table) <br /> d. Hazardous Materials Inventory(attached) <br /> e. Business Activities Page (sign-on to CERS for review) <br /> f. Business Owner/Operator Identification Page (sign-on to CERS for review) <br /> Item#105: CFR 112.1(b)(3), 112.2 Failure to properly close tanks when making a claim of <br /> permanently closed. <br /> Summary of Violations: <br /> • A tank labeled as permanently closed was observed with a hose and pump attached to the <br /> tank. When a tank is not in use, it must be permanently closed by meeting the following <br /> conditions: <br /> o Remove all liquid and sludge from each container and connecting line <br /> o Disconnect and blank off all connecting lines and piping have from the tank <br /> o Close and lock all valves <br /> o Post a sign conspicuously stating that it is a permanently closed container and denoting <br /> the date of closure. <br /> Page 1 of 5 <br />
