CLASS II. or MINOR -Notice to Comply) <br />Onsite to conduct a routine underground storage tank inspection and observe monitoring system <br />certification conducted today. A line test was conducted in lieu of positive shut down. <br />Class 11 Violations: 204, 311, 313 <br />204. The designated operator failed to document all the alarms from the attached alarm history on the <br />March 2014 and June 2014 designated operator monthly inspection reports and failed to check that they <br />were responded to appropriately. The missing alarms include: L10 alarm on February 28, 2014 and May <br />15, May 28 and June 16, 2014 alarms associated with testing conducted during the most recent UDC <br />upgrade. During the monthly inspection, the designated operator shall review the alarm history for the <br />previous month, check that each alarm was documented and responded to appropriately, and attach a <br />copy of the alarm history with documentation taken in response to any alarms to the monthly report. <br />Ensure that designated operators performing monthly inspections at this facility are including all of the <br />required information on the reports. While onsite, the designated operator documented all <br />aforementioned alarms. CORRECTED. <br />311. Both diesel grade (tank 4 and tank 5) line leak detectors failed to detect a leak when tested. All <br />line leak detectors shall be capable of detecting a 3 -gallon per hour leak at 10 psi. Immediately have a <br />properly licensed, trained, and certified contractor repair or replace the failed leak detector (LG 113-x <br />listed, if applicable) and obtain a permit within one business day from the EHD. If the failed leak <br />detectorjcan't be replaced immediately, there is a possibility that diesel UST system may be red <br />tagged to prevent fuel inputs. <br />313. The diesel spill container (located closest to the convenience store, western diesel tank) failed <br />when tested. All spill containers shall have a minimum capacity of five gallons and be capable of <br />containing a spill or overfill until it is detected or cleaned up. Immediately contact a properly licensed, <br />trained, and certified contractor to repair or replace the spill container under permit and inspection of the <br />EHD. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION Y TIME AT EHD'S CURRENT HO RLY RATE. <br />Inspector Re ed By. <br />EHD 23-02 Rev 04/19/12 Paq,13 UST INSPECTION REPORT <br />
