Box 2009 (1601 East Hazelton Avenue) 0 Stockton, California 95201 a <br /> (209)468-3400 <br /> GEN END PROGRAM <br /> TITLE: ENVIRONMENTAL HEALTH DIVISION EMPLOYEES <br /> SUBPOENAED AS WITNESS <br /> POLICX NUMBER 93-005 <br /> EFFECTIVE DATE: JUNE 151 1993 <br /> SUPERSEDES: BED MEMO 1985; EHD POLICY #89-019, 8-1-89; <br /> 3-22-91; PHS MEMO 12-18-91 <br /> APPROVED H ,y�/�l -rG = DATE: <br /> : <br /> San Joaquin County Public Health Services Environmental Health <br /> Division (EHD) employees who are subpoenaed as witnesses to <br /> attend a civil action for job related cases are subject to the <br /> requirements identified in Section 68096. 1 of the California <br /> Government Code. <br /> This code authorizes the County to recover all costs associated <br /> with subpoenas including case preparation, travel, and the time <br /> required at the subpoena location. <br /> The time expended for these activities shall be recorded on a <br /> "Daily Activity Log" in the following manner: <br /> Premise <br /> Address Record ID# Prog. Elem._ SVC CD MIN <br /> Location Prog. ID# if Appropriate 551 Time <br /> applicable EHD Program <br /> The charge for these services shall be assessed at the <br /> $78 .00/hour rate. Actual expenses less than this amount shall be <br /> refunded. Actual expenses more than the amount of the deposit <br /> shall be billed to the requesting party. <br /> The EHD subpoenaed employee shall require a one (1) hour minimum <br /> deposit in advance of accepting the subpoena or the employee will <br /> not be allowed to attend the proceedings. The above procedure is <br /> not applicable where the employee is subpoenaed in a case <br /> involving a governmental agency. <br /> Any deviations or problems associated with this procedure shall <br /> ( be referred over to your immediate supervisor. <br /> A Division of San Joaquin County"cAth Care Serviccs <br />
