To destroy MW -1 by over -boring to a total depth of 56 feet bgs and plugging the boring <br />with neat cement. <br />2. To install a replacement well with 4 inch PVC casing in a 10 inch diameter bore hole <br />with a total depth of 60 feet bgs and a screened interval from 30 feet to 60 feet bgs. <br />The well will be use for ground water monitoring initially and for Dual Phase Extraction <br />(DPE) if necessary. <br />3. To develop and sample the well for chemicals of concern. <br />4. To perform a soil gas investigation collecting soil gas samples from 5 feet bgs from 4 <br />separate on-site locations following the California Department of Toxic Substance <br />Control Guidance for the Evaluation and Mitigation of Subsurface Vapor Intrusion to <br />Indoor Air -Interim Final and the California Environmental Protection Agency Advisory - <br />Active Soil Gas Investigation along with current Agency advisories and guidance. <br />5. To plug all soil borings with neat cement after soil gas samples have been collected. <br />COMMENTS AND CONDITIONS: <br />1.
