The letter gave you ten (10) days to respond. Since we received no response <br />from you, you were mailed a No Response to Request for Information letter dated April 15, <br />1998 which requested response with ten (10) days. You telephoned our office on May 7, <br />1998 and requested faxed copies of the previous letters and agreed to respond immediately. <br />To date, you have not submitted the originally requested forms. Therefore, your claim is <br />deemed ineligible for participation in the Fund (Section 2811.2(n)). <br />If you disagree with this Staff Decision, you may either request review and reconsideration by <br />the Program Manager or you may formally appeal the decision and request a Division <br />_.Decision from the Chief of the Division.
