APP <br />Well Installation and Sampling at California National Guard Field Maintenance Shop <br />• Stockton, California <br />8.0 ACCIDENT REPORTING <br />It is the intent of the OTIE Accident and Crisis Management Program to ensure that each and <br />every employee has a safe and healthful work environment and the knowledge to properly and <br />safely perform his/her work. Besides company employees, the program covers all other workers <br />(temporary, part-time and subcontractor) that OTIE controls, directs or supervises on the job, to <br />the extent these workers are exposed to specific work site and job assignment hazards. If air <br />sampling and monitoring is required, the SSHO will make sampling and monitoring results <br />available to the site personnel. Results from personnel monitoring will be provided to affected <br />personnel. The FTL will track exposure date (man-hours worked) on the project daily and <br />document them in the Daily Production Reports. <br />Damage to vehicles and property other than ordinary wear, tear and operating breakage that <br />involves OTIE employees and equipment constitutes an accident and shall be reported promptly <br />to the employee's supervisor. Accidents and injuries will be investigated immediately by <br />supervisory personnel and results reported on a standard company form (found in Appendix H). <br />In the case of a `near miss' accident or injury, a "Near Miss Occurrence Report" shall be <br />completed by the employee witnessing the incident. Copies of all completed forms should be <br />forwarded to the SHM, the PM, and Base or client representative. The SSHO will prepare any <br />required accident/incident reports with assistance from the persons directly involved. <br />As per EM 385-1-1, report all accidents as soon as possible but not more than 24 hours <br />afterwards to the CO/COR. OTIE shall thoroughly investigate the accident and submit the <br />findings of the investigation along with appropriate corrective actions to the CO/COR in the <br />prescribed format as soon as possible but no later than five working days following the accident. <br />Corrective actions will be implemented as soon as reasonably possible. <br />Contractors are responsible for notifying OSHA when one or more of their employees are <br />seriously injured. As per EM 385-1-1 immediate accident notification is required if the <br />following occur. <br />a. Fatal injury, <br />b. Permanent totally disabling injury, <br />c. Permanent partial disabling injury, <br />d. Three or more persons admitted to a hospital as a result of a single occurrence, or <br />e. Property damage in an amount of $200, 000 or more. <br />Employees shall immediately report all injuries or illnesses sustained in the course of <br />employment to the project PM and the OTIE SHM. If an employee goes to a medical facility <br />under emergency conditions, the SSHO will call the SHM and report the accident. When an <br />employee goes to a medical facility, he/she will inform the medical facility that it is a Workers' <br />Compensation case. In turn the client representative will immediately be notified by the project <br />PM. The SSHO is responsible for completing the Contractor Significant Incident Report (CSIR) <br />and the internal OTIE accident reports. The CSIR will be submitted to the client and the OTIE <br />reports will be submitted to the SHM. <br />After being released from the medical facility, an injured employee will bring into the office (if <br />possible) any medical instructions from the medical facility. If an employee is unable to return to <br />the office and is instructed to go directly home, he/she must call the SHM, at which point an <br />accident report will be taken over the phone. If an employee is given "work restrictions" from a <br />• medical facility, he/she must report to their local business office prior to returning to work. Under <br />CTI E 17 <br />
