SAMPLE RECEIVING POLICY <br /> 1 ). Obtain the following client information and place on customer lab ticket: <br /> Billing Name: <br /> Address: <br /> Phone Number: <br /> Person to Contact: <br /> Report Form Required: State FGL <br /> Is Chain of Custody required? Yes No <br /> 2) . Determine the analyses needed and indicate on customer lab ticket: <br /> A) . Use EPA Method Number or list elements <br /> B) . Determine if preservatives have been added <br /> 3) . Determine turn-around-time requirement: Rush Non-Rush <br /> Indicate on the customer lab ticket and the laboratory work sheet if a <br /> rush is required. The red colored rush stamp is to be used for this <br /> purpose. <br /> 4) . Inspect the sample for the following: <br /> A) . Have holding times been observed and determine if it is possible for <br /> FGL to meet holding times? (See attached holding time requirements) <br /> B) . Is the sample size adequate? <br /> C) . Is the sample container satisfactory? (See attached sample container <br /> requirements) <br /> D) . Note sample condition <br /> 1 ) . Broken/leaking container <br /> 2) . Custody Seal Intact Broken <br /> 3} . Temperature Ambient Chilled <br /> 4) . Check for headspace when appropriate <br /> Make note of any problems with sample condition on the <br /> customers lab ticket, the person notified, time and date <br /> notified and customers response if any. <br /> 5) . Check all samples for radioactivity analyses and hazardous waste <br /> evaluation for radio chemical hazard using the Model 3 Survey <br /> Meter kept in the log-in room. If the sample is found <br /> to have a reading of 0.3 mrads/hour or greater; then, the sample <br /> must be refused. <br /> 5) . Log the sample information into the appropriate sample log-in book. The <br /> following logbooks should be at the log-in counter: Inorganic Drinking <br /> and Wastewater Lab Samples, Organic Lab Samples, Radioactivity Lab <br /> Samples, and Ag Lab Samples. <br /> 5) . Transfer samples and analyses instructions (lab work sheets) to the <br /> appropriate refrigerator or lab work distribution area. See the attached <br /> sheet titled "Sample Storage and Distribution Policy". <br /> -9- <br />
