Use of any item from the first-aid kit shall necessitate completion of an Accident/Injury <br />Report. The report shall be submitted to Health and Safety within 1 working day. <br />5. For local field services work, first-aid kits shall be returned to the storeroom at the end of <br />each workday. <br />6. First-aid kits shall be inspected and restocked weekly. An inventory of first-aid supplies <br />sufficient to restock kits on a weekly basis shall be maintained. <br />7. For jobs outside the local area, the site supervisors shall replenish the kit from the nearest <br />AW pharmaceutical source, with equivalent supplies to those used (until proper restocking by the <br />r <br />storeroom can be accomplished), unless such supplies can economically be made available to <br />the job from the storeroom. <br />8. Personnel permitted to use first-aid kits shall possess a current first-aid card. <br />W 11.3.2 Eyewash Units <br />Eyewash units meeting the requirements of ANSI Standard Z358.1-1990 will be utilized at the site. All <br />units shall be capable of supplying hands-free irrigation for both eyes for at least 15 minutes at a flow rate <br />'-• of at least 0.4 gallons per minute. <br />11.3.3 Fire Extinguisher <br />A fire extinguisher with a minimum rating of 1-A, I OB,C will be available for use at the site at all times. <br />Site personnel will be readily aware of the location of the fire extinguisher at all times in the event of an <br />incident where a fire extinguisher may be utilized. <br />11.4 NOTIFICATION AND RECORDKEEPING <br />Any injury or illness will be immediately reported to the PM/SSO, who will implement any immediate <br />corrective actions and report the incident to the Health and Safety Professional. OSHA requires <br />notification within 24 hours, and preferably during the same work shift, in the event of a fatality or severe <br />injury requiring hospitalization. The Health and Safety Professional will make such notifications to <br />•+ OSHA, and therefore must receive the information in time to make the notification without penalty. <br />11.5 RESPONSE ACTIONS - SAFETY EQUIPMENT PROBLEMS <br />An emergency may develop due to malfunction or other problems associated with health and safety <br />equipment being used by field personnel. These equipment problems must be corrected before <br />proceeding with field activities. Health and safety problems that may occur include: <br />r. <br />1. Leaks or tears in protective clothing <br />2. Failure of respiratory protective devices (SCBA, air -purifying respirators) <br />3. Encountering contaminants for which prescribed protective equipment may not be suitable. <br />11.6 RESPONSE ACTIONS - MEDICAL EMERGENCIES <br />F Medical emergencies can be described as situations that present a significant threat to the health of <br />.. personnel. These can result from chemical exposures, heat stress, cold stress, and poisonous insect or <br />snake bites. Medical emergencies must be dealt with immediately and proper care should be <br />administered.
