D & S Dragline <br />237 Whitmore Ave. PO Box 705 <br />Modesto CA 95358 Los Banos CA 93635 <br />RE: 574 West Grant Line, Tracy CA <br />Underground Storage Tank (UST) Repair Permit <br />San Joaquin County Public Health Services, Environmental Health Division (PHS/EHD) <br />has reviewed information submitted to this agency for the purpose of obtaining an <br />Underground Storage Tank repair permit. In order to continue processing of your permit the <br />following additional information must be submitted: <br />1) The work plan for the UST repair must state that the special inspector, as defined in <br />Section 25281 Chapter 6.7 of the California Health and Safety Code, must enter the <br />UST to assess and certify any repair. Please submit a copy of the Special Inspector's <br />California State Engineers registration and stamp to this agency. <br />2) The workplan must state that the Certified Industrial Hygienist (CIH) shall be on site <br />during tank entry procedures. <br />3) Please submit a site safety plan that specifies that fueling operations must cease <br />during the tank entry procedure. The site safety plan must specify a procedure to <br />properly seal all vents between tanks during entry. The site safety plan shall grant the <br />CIH sole and absolute authority to suspend tank entry procedures if deemed <br />necessary by the CIH. The site safety plan shall include a copy of the California Code <br />of Regulations (CCR), Title 8, Section 5157 Permit Required Confined Spaces, <br />Appendix D- Confined Space Entry Permit. <br />4) The Tracy Fire Department must approve the safety plan for entry. <br />5) The work plan must state that the Underwriters Laboratory Listing of the UST shall <br />be re -certified subsequent to any repairs made on the UST and that the re- <br />certification of the UST must be provided to PHS/EHD in writing upon completion <br />of all repair work. <br />6) The work plan must state that the tank manufacturer shall extend the factory <br />warranty subsequent to any repairs made on the UST. <br />7) Tank entry shall not be made unless the Lower Explosive Limit is G 10%. <br />A Division of San Joaquin County Heaith Care Services <br />
