The Work Plan has been approved with the following conditions: <br />1) The damaged triple rinsed and inerted UST's will be transported as non -hazardous with <br />a completed tank tracking record (EH Form 23,346 encl.) to be returned to PHS/EHD <br />within 30 days of acceptance of the UST by Containment Solutions in Bakersfield CA. <br />The UL number for each UST will be recorded on EH Form 23 046. <br />2) The triple rinsed UST's will be inerted prior to removal using dry ice at an application <br />rate of 30 lbs dry ice for every 1,000 gallons of UST capacity. <br />3) When the percent LEL and Oxygen concentration in the UST's achieve the criteria <br />stipulated by the Tracy Fire Department as safe enough to remove the UST's, the <br />percent LEL and the Oxygen concentration will be recorded on the DTSC Tank Closure <br />Certification Form. The contractor will complete the form and a copy of the certificate <br />form shall be given to the Environmental Health Inspector at the time of UST removal, <br />another copy of the certificate shall accompany the tank to the recycling/disposal facility. <br />4) Tracy Fire has given authorization for the UST removal under City of Tracy permit <br />;r 991649. <br />5) PHS/EI-1D has added the previously permitted 10,000 gallon UST to the repair permit <br />based on the June 30, 1999 letter from Containment Solutions. <br />6) The water pumped from the excavation will be stored on site until it is transported off <br />site by truck under manifest to the Tracy POTW under their approval. <br />A Division of San loaquin County Health Care Service <br />
