Acacia St., Stockton Program: APSA <br /> SUMMARY OF VIOLATIONS <br /> CLASS 1,CLASS II, or MINOR-Notice to Comply) <br /> 33. SPCC Plan did not discuss how to secure master flow & drain valves. <br /> -- SPCC Plan must include descriptions of how you secure and control access to the oil handling, <br /> processing & storage areas, secure master flow & drain valves, prevent unauthorized access to starter <br /> controls on oil pumps, secure out-of-service and loading/unloading connections of oil pipelines, and <br /> address the appropriateness of security lighting to both prevent acts of vandalism and assist in the <br /> discovery of oil discharges. <br /> * Immediately update the SPCC Plan to include all of the required security information and submit a <br /> copy to this office by April 3, 2011. <br /> 35. No mechanism for preventing vehicles from departure before loading/unloading disconnection. <br /> -- The facility must provide an interlocked warning light or physical barrier system, warning signs, wheel <br /> chocks, or vehicle brake interlock system in the loading/unloading area to prevent vehicles from <br /> departing before complete disconnection of flexible or fixed oil transfer lines. <br /> * Immediately provide a means of preventing vehicle departure prior to transfer line disconnection that is <br /> in accordance with 40 CFR Part 112 and your SPCC Plan by April 3, 2011. <br /> 38. SPCC Plan did not include language of conformance to applicable regulations. <br /> -- SPCC Plan shall include a complete discussion of conformance with the applicable requirements and <br /> other effective discharge prevention and containment procedures listed in part 112 or any applicable <br /> more stringent State rules, regulations, and guidelines. <br /> Submit SPCC Plan with discussion of conformance per this section by April 3, 2011. <br /> 57. No routine inspection of valves and piping by the facility. <br /> -- Facility shall regularly inspect all aboveground valves, piping, and appurtenances to access the <br /> condition of items, such as flange joints, expansion joins, valve glands and bodies, catch pans, pipeline <br /> supports, locking of valves, and metal surfaces. <br /> * Immediately begin routine inspection per this section. <br /> 59. No warnings were observed on site to warn vehicles of aboveground piping, containers, or other <br /> appurtenances. <br /> -- A warning must be provided to all vehicles entering the facility to be sure that no vehicle will endanger <br /> aboveground piping or other oil transfer operations. <br /> * Immediately provide a warning in accordance with CFR 40, Part 112 for all vehicles entering the <br /> facility. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EH nspe Received By: Title: <br /> SMR l My 3/z gal <br /> EH 28.0 3 ev08/10N CONTINUATION FORM <br />
