The Spill Prevention, <br /> Control, and Countermeasure(SPCC) Plan shall include a facility diagram which must mark the location and <br /> contents of each fixed storage container and the storage area where mobile or portable containers are located. It <br /> must identify the location of and mark as"exempt" underground tanks. It must also include all transfer stations and <br /> connecting pipes, including intra-facility gathering lines. Immediately update the facility diagram to include all of the <br /> required information. Submit a legible copy of the updated facility diagram to the EHD for review. <br /> This is a repeat violation, Class II. <br /> 611 CFR 112.7(b) Plan failed to include a description of potential equipment failure or provide equivalence. <br /> Potential equipment failure and the resulting spill for the 275 gallon day tank, 150 gallon day tanks and the 100 <br /> gallon day tank were not addressed in the Spill Prevention, Control, and Countermeasure(SPCC)Plan. The SPCC <br /> Plan must include a prediction of the direction, rate of flow, and total quantity of oil which could be discharged from <br /> the facility as a result of major equipment failure. Immediately amend the SPCC Plan to include this information and <br /> submit a copy of the revision to the EHD, or provide equivalence as allowed by 40 CFR 112.7(a)(2). <br /> This is a minor violation. <br /> 613 CFR 112.7(c) Failure to provide appropriate secondary containment, diversionary structures or equipment. <br /> [DESCRIBE WHAT YOU SAW] . A facility shall provide appropriate containment and/or diversionary structures or <br /> equipment to prevent a discharge. The entire containment system, including walls and floor, must be capable of <br /> containing oil and must be constructed so that any discharge from a primary containment system will not escape the <br /> containment system before cleanup occurs. Immediately provide adequate secondary containment for all <br /> aboveground petroleum storage containers larger than 55 gallons. <br /> This is a minor violation. <br /> 628 CFR 112.7(a)(1), 112.70) Failure to include in the Plan discussion of conformance with Federal and State <br /> requirements. <br /> The SPCC plan did not include a discussion on conformance with Federal and State requirements.
