The following Minor Violation - Notice to <br /> Comply was observed. #101 <br /> 101. This facility does not have a valid EPA ID number to manage hazardous waste. A hazardous waste <br /> generator shall not treat, store, dispose of, transport or offer for transportation, hazardous waste without <br /> an EPA ID number. Immediately obtain an EPA ID number to manage hazardous waste and submit <br /> evidence to the EHD by 01/7/2012. An application form has been provided. <br /> NOTES <br /> This facility generated less than 5 tons of hazardous waste in 2010.
