Airport Way <br /> Stockton, CA 95206-3919 <br /> CAMN-'TO-024 <br /> 10 January 1995 <br /> MEMORANDUM FOR To Whom It May Concern <br /> SUBJECT: Memorandum For Record <br /> 1 . On 21 Dec 94 , this OMS had a unannounced "Hazardous Waste In- <br /> spection", from the San Joaquin County Public Health Services, <br /> Environmental Health Division, by Mr. Michael P. Kith. <br /> 2 . This OMS had two (2) storage violations as listed below: <br /> a . Stored hazardous waste on site longer than 90 days without a <br /> permit or authorization. <br /> b. Failed to properly label containers of hazardous waste. <br /> 3 . Ref violation # 1: This shop has forwarded two (2) request to <br /> OTAG and have made several calls to have the excessive waste picked <br /> up. As of this date there has been no action on this matter. <br /> 4 . Ref violation # 2 : All containers were labled, but due to <br /> inclement weather the writing has faded. <br /> 5. On 6 Jan 95 this shop received the the results of the 21 Dec 94 <br /> inspection . Approx 1300 hrs on 6 Jan 95 MSG John V. Marquis called <br /> Mr. Tyrone Smith to inform him that we had we had 10 days to ensure <br /> that all hazardous waste is properly disposed of. Mr Smith was not <br /> in his office and a message was left for him. On 10 Jan 95, MSG <br /> Marquis attempted to contact Mr. Smith, as before he was not in his <br /> office and a message was left. <br /> i <br /> 6. As of 10 Jan 95 we still have waste on hand and we have until <br /> 16 Jan 95 to get rid of it. <br /> GERALD H.
