Aurora St., Stockton <br />Program: HW <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />49. The training program and training records on site are incomplete. Facility personnel shall complete a <br />training program in accordance with Title 22 regulations. This program shall be directed by a person <br />trained in hazardous waste management procedures and shall include the following: <br />- hazardous waste management procedures, including contingency plan implementation <br />- emergency procedures, equipment, and systems <br />- key parameters for automatic waste feed cut-off systems <br />- communications or alarm systems <br />- response to fire, explosion, or groundwater contamination incident <br />- shut down of operations <br />The training program shall be completed by each employee within six months of hire, prior to <br />unsupervised work, and shall be reviewed annually. Training records shall be maintained on site until <br />closure of the facility and shall include: <br />- name & job title of each employee <br />- written job description for each job title listed, including requisite skill, education, qualifications, & duties <br />- description of training to be given to each position <br />- records that training was completed for each employee <br />Immediately prepare and implement a complete hazardous waste management training program for this <br />facility in accordance with Title 22 regulations and ensure that all employees complete this training at <br />least annually. Submit a copy of the training program and records the the EHD by September 23, 2011. <br />59. Manifest numbers 004568314JJK (5125110) and 000324474VES (11118/09) were found on site at the <br />facility but were not on record with the Department of Toxic Substances Control (DISC). Within 30 days <br />of each shipment, a generator shall submit a legible copy of each manifest used to DTSC. Immediately <br />submit a copy of these manifests to DTSC and ensure that copies of all future manifests are submitted <br />to DTSC within the required time frame. <br />60, 61, & 62. Manifest numbers 004568314JJK (5/25/10), 008139601 JJK (3128111), 00046160OVES <br />(4/28/11), 007433535JJK (5/5/11), and 008145008JJK (5/25111) were found without a signed off copy <br />from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off <br />by the destination facility on site for three years and have them readily available for review. If the <br />generator did not receive a copy of the manifest with the handwritten signature of the owner or operator <br />of the facility to which the generator's waste was submitted within 60 days of the date the waste was <br />accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with <br />some indication that the generator has not received confirmation of delivery.
