Page 2 of 2 <br />observation by removing the HAel. • <br />During the facility inspection, it was observed that one steel drum containing PCB ballasts had expired the accumulation <br />date. The steel drum of PCB ballasts were transported by Veolia ES Technical Solutions on 9/01/2011. Please see Exhibit <br />4 -PCB Manifest tracking number 000511944VES. A red steel drum of Used Oil was observed in the appliance recycling <br />area satellite accumulation station with an accumulation start date of January 11, 2011. The Used Oil drum was emptied <br />into a Used Oil Tank and evacuated by Asbury Environmental Services on March 28, 2011, August 16, 2011, and <br />December 28, 2011. Please see Exhibit 31, 35, 36 -. As this drum is routinely evacuated during pickups, we have <br />maintained current dates on all hazardous waste labels. <br />4. Contingency Plan & Training: (45) CCR 66265.52 Contingency Plan incomplete <br />The Contingency Plan onsite at the time of inspection was observed to be missing partial contact information that was <br />unavailable upon request; however, during a revision of the plan, the addition was added to the Emergency Response <br />Binder. This has since been added to the binder and is no longer incomplete. Please see Exhibits 8, 9, 10, 11, 12, 13, 14, <br />15, and 16. "Crew Leader/Emergency Contact: Armando Salgado" was added to both the Emergency Notifications page, <br />and the Incident Communication Plan that are within the onsite contingency plan.
