Contingency Plan & Training: (49) CCR 66265.16 Personnel training records <br />incomplete <br />Personnel Contingency Planning training records from the day of inspection and going <br />forward, have been amended to include Title 22 regulations. All training conducted from the <br />list given on the NOC has included the: Name, Job Title, and Signature of each employee <br />trained. The training curriculum will be continued weekly until complete. Please see Exhibits <br />8, 16, and 17. <br />10. Manifests, B Bills of Lading: (59) CCR 66262.23(a)(4) Failed to send generator manifest <br />copies to the Department within 30 days <br />Manifests: 004568314JJK (5/20/10) was a manifest for an 18 yard bin of Solid Oily Debris. <br />The profile we had on file with the landfill had expired, and was rejected pending new <br />analysis, thus, actual disposal was not made at this time. The profile was approved almost <br />two weeks later and was reissued a new manifest for disposal. Please see manifest <br />006513867JJK. Please see Exhibits 18, and 19. <br />Manifest 000324474VES (11/18/09) was retained onsite and has been resent to the DTSC. <br />Please see Exhibit 20. <br />11. Manifests, 8 Bills of Lading: (60) CCR 66262.40(a) Failed to keep a signed copy for 3 <br />years. Manifests, B Bills of Lading: (61) CCR 66262.42(a) Failed to determine status of <br />hazardous waste when manifest copy is not received. Please see Exhibits 21, 22, 23, and 24. <br />Please note that Armando Salgado, Facility Operations Manager; Diana Costa, Office <br />Manager; and Harry Bauser, Maintenance Supervisor have all received retraining on <br />Department of Transportation (DOT) HM -126F. This course covers General Awareness, <br />including Hazard Classification, Safety, Securing and Loading/Unloading, and Manifest <br />completion. Please see Exhibit 25, 26, and 27 <br />12. Universal Waste: (79) CCR 66273.33(b) Failed to (1) store UW lamps in a closed <br />container and in a manner that prevents breakage and release to the environment, or <br />(2) immediately clean and containerize any broken lamps or releases to the <br />environment <br />The UW lamps observed during the inspection are no longer accumulated in a 55 gallon <br />drum. Training and proper handling of the UW lamps was reviewed by all personnel involved <br />in the recovery process. Please see Exhibit 17. <br />13. Universal Waste: (83) CCR 66273.34 Failed to label universal waste to identify type of <br />universal waste. (a) batteries, (b) Mercury containing equipment, (c) lamps, (d) <br />Electronic devices, (e) CRT's, (t) CRT glass <br />Following the proper storage of the Fluorescent light bulbs, a label was attached to each <br />container that identified its contents as 'Universal Waste", its specific contents, accumulation, <br />etc. <br />
