CONTINUATION FORM Page: 2 of 2 <br /> FICIAL INSPECTION REPORT Date: 10/10/05 <br /> Facility Address: 29 N. Aurora Street---------------MEPCO LABEL SYSTEMS Program: 2220 <br /> REPORT OF VIOLATION <br /> Note: A Hazardous Waste Inspection was conducted at your facility on 1/11/05. At that time <br /> two samples were taken from your pre-treatment wash and your dried filter cake. Sample <br /> results were received by this department on 4/6/04 and found to be above hazardous waste <br /> levels. A Phone call was placed to your facility informing you of these results on this day. <br /> On 4/12/05 Mr. John Jackson, Registered Environmental Health Specialist (REHS), and Mr. <br /> Leo Valdez, Hazardous Substances Scientist with the Department of Toxic Substances Control, <br /> went to your facility and explained the findings of the sample results with Tom Gassner. <br /> Instructions were given on how to handle the filter cake as a hazardous waste in accordance <br /> with Title 22 California Code of Regulations. <br /> #4. Health & Safety Code (HSC) section 25189.5, Disposed or caused disposal of hazardous <br /> Waste at an unauthorized facility. (Class 1 Violation) -------------------------------------------------------------- <br /> This facility threw its' filter cake in the trash. Stop this process immediately. Handle all <br /> hazardous wastes in accordance with all applicable Federal and State statutes and regulations. <br /> Start Immediately. <br /> Thank you, <br /> John Jackson, REHS, San Joaquin County Environmental Health Department <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> SAN JOAQUIN COUNTY ENVIRONMENTAL HEALTH DEPARTMENT-304 E WEBER AVE, STOCKTON,CA 95202 (209)468-3420 <br />
