The following is an itemized list of hazardous waste violations that have not been <br /> % addressed for TRACY CHEVROLET as of April 17, 2018. <br /> Open violations from December 05, 2017 inspection <br /> Violation#102 -Failed to determine if a waste is a hazardous waste. <br /> On 11/6/2015, truck washout from maintenance shop was hauled away as non hazardous waste. Any person who <br /> generates a waste shall determine if the waste is a hazardous waste. Immediately stop hauling truck washout as <br /> non hazardous waste, make a hazardous waste determination for the truck washout, and manage it according the <br /> Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste <br /> will be managed. <br /> Violation#108 -Contingency plan is incomplete. <br /> The facility's contingency plan is incomplete. The contingency plan is lacking locations of emergency equipment and <br /> evacuation routes for facility personnel. The contingency plan must include: <br /> 1. Description of actions facility personnel will take in response to fires, explosions, or any sudden or <br /> non-sudden release of hazardous waste to air, soil or surface water at the facility; <br /> 2. Description of arrangements made with local police departments, fire departments, hospitals, contractors and <br /> State and local emergency response teams to coordinate emergency services; <br /> 3. List of names, addresses, and phone numbers (office/home/cell) of all persons qualified to act as emergency <br /> coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and <br /> all others in order in which they will assume responsibilities; <br /> 4. List all emergency equipment, which includes location, physical description, and brief outline of its capabilities; <br /> 5. Evacuation routes for facility personnel where there is a possibility of evacuation; <br /> 6. Current telephone number for State Office of Emergency Services. <br /> If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency <br /> plan, the owner or operator need only amend that plan to incorporate hazardous waste management provisions that <br /> are sufficient to comply with the requirements of this chapter. Immediately prepare or amend an existing plan to <br /> fulfill the requirements of this chapter. Submit a copy of the contingency plan to the EHD. <br /> Violation#502 -Failed to comply with new hazardous waste tank system assessment requirements. <br /> Facility has a tank assessment for the 480 gallon the Waste Oil tank conducted on June 22, 2011. Hazardous waste <br /> tank assessments must be conducted every five years. The owner or operator of a hazardous waste tank shall <br /> obtain a written assessment reviewed and certified by an independent, qualified, professional engineer, registered in <br /> California attesting that the system has sufficient structural integrity, is acceptable for the transferring, storing, and <br /> treating of hazardous waste, and that the tanks and containment system are suitably designed. Immediately have <br /> an assessment prepared for the 480 gallon the Waste Oil tank. Submit a schedule showing when this will be <br /> accomplished to the EHD and submit a copy of the tank assessment to EHD. <br /> Violation #507 -Failed to conduct daily inspections of hazardous waste tank system and maintain records <br /> onsite. <br /> Inspections of the hazardous waste tank system was not performed and documented. The following items are <br /> required to be inspected at least daily: <br /> 1. overfill/spill control equipment; <br /> 2. aboveground portions of the tank system to detect corrosion or releases of water; <br /> 3. data gathered from monitoring and leak-detection equipment; <br /> 4. outside areas of the tank system including secondary containment to detect erosion or signs of releases of <br /> hazardous waste; <br /> 5. for uncovered tanks, the level of waste in the tank. <br /> Immediately begin required inspections of the hazardous waste tank system, and document the items inspected. <br /> Submit a copy of the inspection log for the next 30 days to the EHD. <br /> Page 1 of 3 <br />
