The following is an itemized list of hazardous waste violations that have not been % addressed for TRACY CHEVROLET as of April 17, 2018. Open violations from December 05, 2017 inspection Violation#102 -Failed to determine if a waste is a hazardous waste. On 11/6/2015, truck washout from maintenance shop was hauled away as non hazardous waste. Any person who generates a waste shall determine if the waste is a hazardous waste. Immediately stop hauling truck washout as non hazardous waste, make a hazardous waste determination for the truck washout, and manage it according the Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste will be managed. Violation#108 -Contingency plan is incomplete. The facility's contingency plan is incomplete. The contingency plan is lacking locations of emergency equipment and evacuation routes for facility personnel. The contingency plan must include: 1. Description of actions facility personnel will take in response to fires, explosions, or any sudden or non-sudden release of hazardous waste to air, soil or surface water at the facility; 2. Description of arrangements made with local police departments, fire departments, hospitals, contractors and State and local emergency response teams to coordinate emergency services; 3. List of names, addresses, and phone numbers (office/home/cell) of all persons qualified to act as emergency coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and all others in order in which they will assume responsibilities; 4. List all emergency equipment, which includes location, physical description, and brief outline of its capabilities; 5. Evacuation routes for facility personnel where there is a possibility of evacuation; 6. Current telephone number for State Office of Emergency Services. If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency plan, the owner or operator need only amend that plan to incorporate hazardous waste management provisions that are sufficient to comply with the requirements of this chapter. Immediately prepare or amend an existing plan to fulfill the requirements of this chapter. Submit a copy of the contingency plan to the EHD. Violation#502 -Failed to comply with new hazardous waste tank system assessment requirements. Facility has a tank assessment for the 480 gallon the Waste Oil tank conducted on June 22, 2011. Hazardous waste tank assessments must be conducted every five years. The owner or operator of a hazardous waste tank shall obtain a written assessment reviewed and certified by an independent, qualified, professional engineer, registered in California attesting that the system has sufficient structural integrity, is acceptable for the transferring, storing, and treating of hazardous waste, and that the tanks and containment system are suitably designed. Immediately have an assessment prepared for the 480 gallon the Waste Oil tank. Submit a schedule showing when this will be accomplished to the EHD and submit a copy of the tank assessment to EHD. Violation #507 -Failed to conduct daily inspections of hazardous waste tank system and maintain records onsite. Inspections of the hazardous waste tank system was not performed and documented. The following items are required to be inspected at least daily: 1. overfill/spill control equipment; 2. aboveground portions of the tank system to detect corrosion or releases of water; 3. data gathered from monitoring and leak-detection equipment; 4. outside areas of the tank system including secondary containment to detect erosion or signs of releases of hazardous waste; 5. for uncovered tanks, the level of waste in the tank. Immediately begin required inspections of the hazardous waste tank system, and document the items inspected. Submit a copy of the inspection log for the next 30 days to the EHD. Page 1 of 3
