ATTACHMENT C <br /> 5. Reporting <br /> a. Obligation to Report: Depending upon the volume involved, the dealership may be required to <br /> report a release or threatened release of a hazardous material. No report is required for small, <br /> non-harmful releases such as a quart of drain oil on the shop floor. <br /> b. Reporting Authority: Employees who detect a release or threatened release of a hazardous <br /> material (other than an incidental spill) should immediately notify their supervisor, who shall <br /> then be responsible for assessing and beginning a response to the hazard posed. If the <br /> release/threatened release involves a significant volume of hazardous material, the supervisor <br /> should promptly notify one of the following who shall have reporting authority: <br /> ■ Service Manager <br /> ■ Parts Manager <br /> ■ Dealer Principal <br /> If none of these parties can be notified,the supervisor has the authority to make the report. <br /> e. Reporting Procedure: If uncertain as to whether a report is required, the responsible individual <br /> should first query one of the below listed agencies. If it is determined that a report is required, it <br /> should be made to the following: <br /> ■ EPA National Response Center-(800)424-8802 <br /> ■ Emergency Fire/Police-911 <br /> ■ California Emergency Services Warning Center-(800) 852-7550 <br /> • San Joaquin County Department of Enviornmental Health -(209)468-3420 <br /> The report should be made as soon as possible as long as it does not impede immediate control <br /> of the release or interfere with emergency medical measures and should include the following <br /> about the release or threatened release: <br /> ■ The source and cause of the release <br /> ■ The exact location and phone number of the facility <br /> ■ The name of the person that made the report <br /> ■ The type and quantity of hazardous material(s) involved <br /> ■ The potential hazard(s) presented(if known) <br /> ■ Any damage or injuries caused by the release <br /> ■ Actions taken to control or mitigate the release <br /> • Names of other individuals or organizations that have been contacted <br /> The report should follow the format of the Reporting Guide Procedure that contains the <br /> pertinent information noted above. <br /> Appropriate emergency response agencies and dealership managers should also be notified. <br /> d. Written Record: A written record of all verbal notifications should be prepared by the reporting <br /> individual and provided to the dealer principal. This report may be handwritten and should <br /> include the following: <br /> ■ Date and time of call <br /> ■ Person making call <br /> ■ Agencies notified <br /> ■ Individuals contacted <br /> • Summary of conversation(s) <br /> Tracy Chevrolet SPCC Plan <br /> 08-14 <br />
