Also we have taken extra precautionary steps by hiring an <br /> outside contractor, Tanlmology to perform a refresher course to all our Market Managers, to <br /> insure they are properly conducting the DO inspection and completing the forms throughly. <br /> Issue#4: <br /> Last noted employee training was conducted on June 10,2008. Employees must be trained <br /> within 30 days of hire and provided a refresher annually. <br /> Response: <br /> New and annual training of store employees were conducted by Rudy Tovar,the Market Manager <br /> for the location. <br /> Issue#5: <br /> Annual Monitor Certification,line leak detector, and spill bucket testing was not found while the <br /> inspection took place. <br /> Response: <br /> Attached is the 2010 Monitor Certification, including line leak detector and spill bucket testing <br /> that was conducted on October 22,2009. It has also been email and mailed to the site location. <br /> Issue#6: <br /> Failed to properly label containers of hazardous waste. <br /> Response: <br /> All hazardous waste drums were picked up June 26, 2009. Attached is a copy of the Hazardous <br /> Waste Manifest. Rudy Tovar, the Market Manager, for the location has been instructed to <br /> properly label the containers once waste has been placed in the drum(s). <br /> Issue#7: <br /> Failed to keep sign copy for 3 years. <br /> Response: <br /> Attached are signed copies of the site's waste manifests.This has also been sent to site location <br /> via email and USPS. <br /> Issue#8: <br /> Failed to keep a copy of the consolidated manifest for 3 years. <br /> Response: <br /> We have instructed the site's Manger and Market Manager that they need to maintain a log of all <br /> 50 gallon drums and 5 gallon buckets that contain hazardous material,both solid and liquid for 3 <br /> years. <br /> Issue#9: Failed to retain bill of lading for filters for 3 years. <br />
