Training records on former employees shall be kept for at least three years from the date the employee last worked at the facility. Submit a copy of your training records and document to EHD by August 27, 2011. 84. No 2009 disposal record for Universal Waste (UW) fluorescent lamps; facility stated the 2009 waste lamps were stored over 1 year and were disposed in 2010. Universal Waste, including but not limited to lamps, alkaline batteries, mercury containing switches, CRT, not empty aerosol cans, and electronic devices shall be stored on site no longer than 1 year. Begin managing all universal wastes within 1 year of storage, and retain a copy of the disposal receipt for at least 3 years on site. NOTES (The following are not violations) The facility generated 18.7775 tons (37,555 pounds) hazardous waste in 2010. Facility is an agricultural trucking operation that generates hazardous wastes from truck maintenance. -- Facility has an on-site 3-stage clarifier for a truck-wash that goes to an evaporation pond. In 2010 the liquid from the clarifier was disposed under hazardous waste manifest, but was not disposed as hazardous waste in 2009 or 2011. Previous analytical results from 2003 indicated the liquid to be non- hazardous. Please be aware that as a generator you are responsible for making a hazardous waste determination of each waste and manage it in accordance to that determination. 79. One box of universal waste fluorescent lamps were not closed and has no accumulation start date. Universal Wastes shall be stored in a closed container that prevents breakage and release to the environment and marked with date of accumulation. Discrepancy was corrected on site. A copy of Auto Repair Management Practice factsheet, CUPA class schedule, blank hazardous waste labels, hazardous waste labeling factsheet, tank assessment fact sheet has been provided. Complete and submit a copy of the Return to Compliance Certification form (provided) with a statement of how each violation was corrected, including supporting documentation, by August 27, 2011. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122).
