Failed to keep signed copy for three years. <br />Upon reviewing the records on site, signed off hazardous waste manifests (TSDF) were not found for the <br />following hazardous waste manifests: <br />001738243FLE from 7/31/08 <br />003779015JJK from 4/24/08 <br />003779016JJK from 4/24/08 <br />003779018JJK from 4/25/08 <br />Signed off hazardous waste manifests must be retained on site for the last three years. Provide copies <br />of the missing signed off hazardous waste manifest copies to the EHD by October 21, 2010. <br />Submit the return to compliance certification with a corrective actions statement and supporting <br />documentation by October 21, 2010. <br />Note: The following violation was corrected on site: Failed to make a hazardous waste determination for <br />used cutting oil located in the welding shop. The hazardous waste generated for 2009 was <br />approximately 3.4 tons. The permit level will remain the at the same level of less than 5 tons/yr.
