An emergency coordinator and modified contingency plan information is lacking. There must be <br />at least one emergency coordinator on site or on call to coordinate emergency response measures, and <br />the following information must be posted by a phone: the name and phone number of the emergency <br />coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br />phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an <br />emergency coordinator and post the required information by a phone. A form is provided that can be <br />used for this purpose. Submit proof of correction to the EHD by December 8, 2013. <br />804. Failed to store universal waste lamps in a closed container. 15 open boxes (40 tubes per box) of <br />waste fluorescent tubes were observed in the lighting storeroom. Place all universal waste fluorescent <br />tubes in closed containers adequate to prevent breakage. Correct immediately. <br />808. Failed to label universal waste tamps. 15 open boxes (40 tubes per box) of waste fluorescent <br />tubes in the shipping/receiving area were not labeled. Label all containers of universal waste lamps with <br />the words: "universal waste - lamps". Correct immediately. <br />809. Facility was unable to demonstrate the length of time the universal waste fluorescent tubes have <br />been stored on site. The facility does not have recycling records or receipts showing proper recycling of <br />universal waste. Recycle the universal waste on site including the fluorescent tubes and send the <br />recycling receipt to the EHD showing that the universal waste was sent to an universal waste handler. <br />Note <br />1.
