FIVED <br /> San Joaquin County AMENDED <br /> QED <br /> DEC 0 4 2015 Environmental Health Department I ou law(5 <br /> 1868 East Hazelton Avenue, Stockton,California 95205.6232 <br /> ENV!RCNME~one: (209)468-3420 Fax:(209)468-3433 Web:www.sioov.org/ehd <br /> H arN' uantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Lawrence Livermore National Lab-Site 300 1 15999 W CORRAL HOLLOW RD Trac September 30 2015 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS Il,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 301 CCR 66265.31 Facility not maintained to minimize the release of a hazardous waste. <br /> The following observations were made during the inspection: <br /> -At building 8276, metal fines generated from sheet metals were observed on and around the table vise,belt sander, <br /> and work table in the mechanic shop, <br /> -At building 843, metal fines were observed in the outdoor working area around a machine labeled"Metal Only." <br /> According to Larry Griffin, the metal fines are generated from steel.A damaged pallet of well pumps was leaking oily <br /> liquid onto the ground outside of building 843. <br /> -At building 874, metal fines were observed on the floor and belt sander. <br /> -At south of building 875, a heavy equipment leaking oil, partially onto two drip pans and partially on the ground,with <br /> dark oily stained ground was observed. <br /> -At building 875, metal fines were observed on the floor and various equipments inside the shop. <br /> According to Mr. Newman,all metal fines are handled as hazardous waste. Facilities shall be maintained and <br /> operated to minimize the possibility of a fire, explosion,or release of hazardous waste to air,soil, or surface water <br /> which could threaten human health or the environment Immediately correct all items listed above and manage <br /> according to Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how <br /> this waste was managed. <br /> This is a Class II violation <br /> 303 CCR 66265.33 Failed to maintain all communication or alarm systems, spill control, or decontamination equipment. <br /> The following observations were made during the inspection: <br /> -At building 843, a damaged pallet of well pumps containing oily liquid was observed outside. <br /> -At building 874,the eyewash station was blocked by a large piece of work equipment <br /> -At upper Corp Yard 875,white residue was observed in the secondary containment Inside the shed. <br /> All facility communications or alarm systems,fire protection equipment, spill control equipment, and decontamination <br /> equipment,where required, shall be tested and maintained as necessary to assure its proper operation in time of <br /> emergency. Immediately begin this practice. <br /> This is a Class II violation. <br /> 401 CCR 66265.171 Failed to store hazardous waste in a container that is in good condition. <br /> Found a 2 gallon broken white container labeled as chlorine with about 15 grams of white solids at 875 lower Corp <br /> Yard. Hazardous waste shall be stored in a container that is in good condition.
