This facility utilizes a site <br />specific record keeping software to maintain the required training records. One of the reviewed training records <br />generated by this system, indicated that one employee (Mr. Adam Conour) had an expired (unfulfilled) initial training <br />for a Hazardous Waste Retention Tank Operator course EP001-001, which was due on 7-8-2016. Mr. Paul Roy and <br />Mr. Stan Terusaki indicated that even though this training was marked as required by their system, it was not <br />required for Mr. Conour's position. As such, the onsite record keeping software does not accurately reflect the <br />up-to-date written description of the type and amount of both introductory and continuing training that will be given to <br />each employee. <br />Employee training records shall include: <br />1. the job title for each position at the facility related to hazardous waste management, and the name of each <br />employee filling each job; <br />2. written job description for each position listed above; <br />3. written description of the type and amount of both introductory and continuing training that will be given to each <br />person filing positions listed above; <br />4. records that document that the training has been given to and completed by facility personnel. <br />Training records on current employees shall be kept until closure of the facility. Training records on former <br />employees shall be kept for at least three years from the date the employee last worked at the facility. Ensure that <br />employee training is properly documented. Provide proof of correction that all required training records are being <br />maintained. <br />This is a repeat violation, Class ll. <br />123 r HSC 25143 10 Failed to provide information for >100 kglmo recyclable material to claim the exclusion or exemption <br />Used oil filters and lead acid batteries are being handled as excluded recyclable materials and all required <br />information to claim the exclusion or exemption has not been submitted. To claim an exclusion or exemption from <br />classifying this as a waste, the following information shall be submitted every two years: <br />- name, site address, mailing address, and telephone number of the owner or operator of the facility that recycles <br />the material; <br />- name and address of the generator of the recyclable material; <br />- documentation that the requirements of any exemptions or exclusions pursuant to HSC 25143.2 are met. <br />immediately provide all required information for every recyclable material that you wish to claim the exclusion or <br />exemption and provide that information every two years. If the required information cannot be provided, the <br />materials must immediately be handled as a hazardous waste.
