REGEIVFn <br /> San Joaquin County yF� ft" - <br /> EEC 0 4 2015 Environmental Health Department �i ��DI 0 <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 to t0�lS <br /> ENVIRONMEWIAtone: (209)468-3420 Fax:(209)468-3433 Web:www.sjgov.org/ehd <br /> IT <br /> arge uantity Hazardous Waste Generator inspection Report <br /> Facility Name: Facility Address: Date: <br /> Lawrence Livermore National Lab-Sita 300 15999 W CORRAL HOLLOW RD Trac Se tember 30 2015 <br /> Item# Remarks <br /> 106 CCR 66262.11 Failed to determine if a waste is a hazardous waste. <br /> The following observations were made during the inspection: <br /> -At building 802,three 5 gallon waste containers of unknown inside the basement area. <br /> -At building 810 Bay 1, solid waste including wipes and Q-tips was observed in a 55 gallon drum that was identified by <br /> the building operator as a Form 4 hazardous waste container. <br /> -At Corp Yard 843,two 55 gallon drums and one open 5 gallon container of Iron filings were observed. <br /> -At building 8556, a 55 blue drum of purple oily liquid was observed by Hazardous Material inspectors. <br /> -At Corp Yard north of 865, eleven 55 gallon drums of unknown including one drum found open with an unknown <br /> yellowish solid nearby burnt debris. <br /> -At 875 upper Corp Yard, there was unknown standing liquid,which was identified by Mr. Stanley Terusaki as being <br /> not just rain water, in secondary containment of the Frion/AC Shop shed. <br /> -At 875 lower Corp Yard, a steel container of unknown liquid labeled as"Chemsearch"and 5 gallon container labeled <br /> as"WT-500"found in shed. <br /> -At south of building 875, one 55 gallon drum of unknown solid nearby a heavy equipment outside. <br /> Any person who generates a waste shall determine if the waste is a hazardous waste. Immediately make a <br /> hazardous waste determination for the above listed items, and manage them according to the Title 22 hazardous <br /> waste regulations. Submit a statement and supporting documentation explaining how this waste was managed. <br /> This is a Class II violation <br /> 108 CCR 66265.52 Contingency plan is incomplete. <br /> The facility's contingency plan is incomplete and/or not current. The contingency plan provided at the time of <br /> inspection listed Livermore Police Department and Alameda County Sheriff as local arrangements made. According to <br /> John Scott,when asked if Livermore Police Department will respond to this facility, he responded"absolutely not." <br /> The contingency plan must include: <br /> 1. Description of actions facility personnel will take in response to fires, explosions, or any sudden or non-sudden <br /> release of hazardous waste to air, soil or surface water at the facility; <br /> 2. Description of arrangements made with local police departments, fire departments, hospitals, contractors and <br /> State and local emergency response teams to coordinate emergency services; <br /> 3. List of names, addresses,and phone numbers(office/home/cell)of all persons qualified to act as emergency <br /> coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and all <br /> others in order in which they will assume responsibilities; <br /> 4. List all emergency equipment, which includes location, physical description, and brief outline of its capabilities; <br /> 5. Evacuation routes for facility personnel where there is a possibility of evacuation; <br /> 6. Current telephone number for State Office of Emergency Services. <br /> If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency <br /> plan, the owner or operator need only amend that plan to Incorporate hazardous waste management provisions that <br /> are sufficient to comply with the requirements of this chapter. Immediately prepare or amend an existing plan to fulfill <br /> the requirements of this chapter.
