SANJ Q a O U I N Environmental Health Department <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> A ADVANCED AUTO BODY & COLLISION 23922 S DARRIGO RD, TRACY March 12, 2018 <br /> omp ete and submit a copy of the Return to Uompliance Uertitication form to the with a s atement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by April 11, 2018. <br /> Waste Streams: used oil, paint filter <br /> Documents received by operator: Modified contingency plan, hazardous waste labels, hazardous waste label <br /> sample, household hazardous waste brochure, Free CUPA class notification page, EPA ID information, <br /> inspection report, and return to compliance certification form. <br /> Note: Managed a hazardous waste without an identification number. This facility does not have a valid ID <br /> number to manage hazardous waste. A hazardous waste generator shall not treat, store, dispose of, transport <br /> or offer for transportation, hazardous waste without an ID number. If you or your business generates more than <br /> 100 kg of RCRA hazardous waste and'or more than one kg of acutely hazardous waste, then you need to obtain <br /> an EPA ID number. All others must obtain a State ID number. Go to the DTSC web site <br /> hftp://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to apply for an ID number. Immediately obtain <br /> an ID number to manage hazardous waste and submit evidence to the EHD. <br /> Note: No modified contingency plan. An emergency coordinator and modified contingency plan information is <br /> lacking. There must be at least one emergency coordinator on site or on call to coordinate emergency response <br /> measures, and the following information must be posted by a phone: the name and phone number of the <br /> emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br /> phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an emergency <br /> coordinator and post the required information by a phone. A form is provided that can be used for this purpose. <br /> Note: Employee Training At the time of inspection, it could not be demonstrated(not necessarily documented) <br /> that employees who handle hazardous waste were properly trained Antonio Del Toro was asked how to <br /> handle, recognize, and label hazardous waste during time of inspection and he could not provide some basic <br /> hazardous waste training However, he was able to explain how to clean up a spill The generator must ensure <br /> that all employees who handle hazardous waste are thoroughly familiar with proper waste handling and <br /> emergency procedures. Provide proof of training to the EHD for employees whose responsibilities include <br /> hazardous waste. <br /> Note: Failed to report annual data electronically. Please be aware as of January 1, 2013, all businesses are <br /> required to submit all hazardous materials information online to the California Environmental Reporting System <br /> (CERS) at http://cers.catepa.ca.gov. Be sure to include your hazardous waste activity in the Businesses <br /> Activities section in CERS in addition to any other relevant activities and required fields. <br /> Note: Failed to maintain required emergency equipment. Fire extinguishers were found on site but they are <br /> expired and spill control kit was not available. All facilities shall be equipped with an internal communication or <br /> alarm system, a device capable of calling outside emergency help, spill control equipment, and decontamination <br /> equipment at all times.
