PUBLIC F'EUTH SERVICES <br />SAN JOAQUIN COUNTY <br />ENVIRONMENTAL HEALTH DMSION <br />Karen Furst, M.D., M.P.H., Health Officer <br />304 East Weber Avenue, Third Floor • Stockton, CA 95202 <br />209/468-3420 <br />NOTICE <br />To: Small Quantity Hazardous Waste Generator Facilities <br />From: Environmental Health Division <br />Date: May 25, 1999 <br />Re: Hazardous Waste Generator Program and State Facility Surcharge Fees <br />On June 25, 1998, the Board of Supervisors adopted a revised Hazardous Generator Fee <br />Schedule and established a new fee category for Small Hazardous Waste Generator facilities <br />(generating less than five tons per year). The enclosed billing statement includes the 1999 Small <br />Hazardous Waste Permit fee of $100. <br />As the Certified Unified Program Agency (CUPA) for San Joaquin County, the Environmental <br />Health Division (EHD) is also required to collect a state facility surcharge fee for each facility <br />regulated under the Hazardous Material Release Response Plans, Hazardous Material <br />Management Plans, California Accidental Release Prevention Program (Cal -ARP), <br />Hazardous Waste Generator Program, Tiered Permitting Program or Underground <br />Storage Tank (UST) Programs. If our records indicate that your facility was not previously <br />billed for the state facility surcharge of $18.50 for 1998 and/or $10.00 for 1999, these charges <br />are included on this billing statement. <br />If used oil is the only hazardous waste generated by your facility, you may be eligible for a 50% <br />reduction in the Small Hazardous Waste Generator Permit Fee next year. Please complete the <br />"Used Oil Only" statement on the bottom of this letter and return it with your payment.
