SUMMARY OF VIOLATIONS (CLASS I,CLASS 11,or MINOR-Notice to Comply) ROUTINE HAZARDOUS WASTE GENERATOR INSPECTION No violations at this time. Notes: (corrected on site) - A 5 gallon bucket containing a pourable amount of motor oil and two black plastic containers partially full of motor oil were found disposed of in the trash can in the building for the waste dirt. The disposal of any hazardous waste, or the causing of the disposal, is prohibited when the disposal is at a facility which is not permitted for hazardous waste disposal. All three containers were removed from the trash for disposal with the used oil at the time of inspection. - Manifest number 009572933JJK (5/15/12) was found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
