EI Dorado St., Stockton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> 49. The training records for the facility included only the employee name and a record that training was <br /> completed. Facility personnel shall complete a training program in accordance with Title 22 regulations. <br /> This program shall be directed by a person trained in hazardous waste management procedures and <br /> shall include the following: <br /> - hazardous waste management procedures, including contingency plan implementation <br /> - emergency procedures, equipment, and systems <br /> - key parameters for automatic waste feed cut-off systems <br /> - communications or alarm systems <br /> - response to fire, explosion, or groundwater contamination incident <br /> - shut down of operations <br /> The training program shall be completed by each employee within six months of hire, prior to <br /> unsupervised work, and shall be reviewed annually. Training records shall be maintained on site until <br /> closure of the facility and shall include: <br /> - name &job title of each employee <br /> - written job description for each position (job title) listed, including requisite skill, education, <br /> qualifications, and duties <br /> - description of training to be given to each position <br /> - records that training was completed for each employee <br /> Immediately prepare and implement a complete hazardous waste management training program for this <br /> facility in accordance with Title 22 regulations and ensure that all employees complete this training at <br /> least annually. Submit a copy of the training records the EHD by June 26, 2011. <br /> 60 & 63. Copies of hazardous waste disposal records for the parts washer waste were not found on site. <br /> Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all <br /> receipts used in a consolidated manifesting procedure on site for three years and have them readily <br /> available for review. Immediately locate a copy of all manifests and receipts for the parts washer waste <br /> for the last three years, maintain them on site, and submit copies to the EHD by June 26, 2011. <br /> 64. Copies of spent lead acid battery disposal records were not found on site. The generator shall retain <br /> on site for at least three years a legible copy of each manifest or bill of lading which identifies spent lead <br /> acid batteries shipped to a person who stores, uses, reuses, recycles, or reclaims the batteries or their <br /> components. Immediately locate a copy of all disposal records for spent lead acid batteries for the last <br /> three years, maintain them on site, and submit copies to the EHD by June 26, 2011. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: ceived By: Title: <br /> EHD 23 2-003Xev 08/10/10 CONTINUATION FORM <br />
