The records did not include a <br /> record that training was conducted in 2013, 2014, or 2015. According to Ms. Dias, hazardous waste training is <br /> conducted several times a year during several of the other similar training topics. The records did not include a record <br /> that the person conducting the training, Mary Dias, was trained at the time. The records did not include the type of <br /> training required for each position. Hazardous waste training shall be directed by a person trained in hazardous waste <br /> management procedures. Employee training records shall include: <br /> 1. the job title for each position at the facility related to hazardous waste management, and the name of each <br /> employee filling each job; <br /> 2. written job description for each position listed above; <br /> 3. written description of the type and amount of both introductory and continuing training that will be given to each <br /> person filing positions listed above, <br /> 4. records that document that the training has been given to and completed by facility personnel. <br /> Training records on current employees shall be kept until closure of the facility. Training records on former employees <br /> shall be kept for at least three years from the date the employee last worked at the facility. At a minimum, the <br /> training program shall be designed to ensure that facility personnel are able to respond effectively to emergencies by <br /> familiarizing them with emergency procedures, emergency equipment, and emergency systems, including where <br /> applicable: <br /> 1. procedures for using, inspecting, repairing, and replacing facility emergency and monitoring equipment, <br /> 2. key parameters for automatic waste feed cut-off systems; <br /> 3. communications or alarm systems, <br /> 4, response to fires or explosions, <br /> 5. response to ground-water contamination incidents, and <br /> 6. shutdown of operations. <br /> Training shall include contingency plan implementation and hazardous waste management duties relevant to the <br /> positions in which they are employed. Ensure that employee training is properly documented. <br /> This is a repeat violation, Class II. <br /> 303 CCR 66265.33 Failed to maintain all communication or alarm systems, spill control, or decontamination equipment. <br /> The eyewash station in the shop was observed with a liquid level several inches below the "15 minute line". <br /> According to the instructions on the eyewash, the station must be refilled when the solution is below the 15 minute <br /> line. All facility communications or alarm systems, fire protection equipment, spill control equipment, and <br /> decontamination equipment, where required, shall be tested and maintained as necessary to assure its proper <br /> operation in time of emergency.
