Hwy 88 Lockeford, Ca 95237 <br />Program: HW <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR -Notice to Comply) <br />On site to conduct routine inspection. Class 2: #20, Minor -Notice to Comply: #50 <br />Class 2 Violation: #20 <br />1 observed that this facility has three 55 gallon drums. Two of the 55 gallon drums had blank hazardous <br />waste labels on them. The black 55 gallon drum did not have identified on it the physical state or the <br />hazardous property of the hazardous waste. Containers of hazardous waste must be labeled with the <br />following: 1. the words, "hazardous waste", 2. generator name and address, 3. contents, 4. physical <br />state, 5. hazardous property, 6. accumulation start date. Faisal Baz, Manager corrected this violation <br />during the inspection. <br />Minor -Notice to Comply: #50 <br />1 observed that the two 55 gallon drums that had blank hazardous waste labels, also lacked <br />accumulation start dates. Faisal Baz, Manager stated that this waste has been accumulating for over a <br />year. Small Hazardous Waste Generators must dispose of their hazardous waste within 180 days of the <br />accumulation start date. Correct immediately. Submit copy of the disposal manifest for this hazardous <br />waste to this office by 6-11-10. <br />Complete and submit the return to compliance certification, corrective action statements, and any <br />supporting documentation to this office by 6-11-10. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($115). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY T T EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Received By: ;_ <br />Title: <br />M <br />
