Multiple attempts have been made by <br /> This information was required to be sub <br /> the EHD in order to obtain the required return to compliance documentation, however, no return to <br /> compliance documentation have been received to date. <br /> Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is <br /> billed at the current hourly rate ($125). Failure to submit the completed information immediately <br /> may result in a re-inspection, additional violations, and/or further legal action. <br /> The missing information includes: <br /> 1) Return to Compliance Certification Form (copy attached hereto) <br /> 2) Corrective Action Statement <br /> 3) Violation 101: EPA ID number is inactive <br /> 4) Violation 108: an .unlabeled five gallon container of used diesel fuel filters and liquid was <br /> observed onsite <br /> 5) Violation 122 and 125: no disposal records for years 2010 through 2013 were available for review <br /> 6) Violation 127: no manifest of bills of lading for the recycling of spend lead acid batteries were <br /> available for review <br /> the recycling of used oil filters were available for review <br /> 7) Violation 128: no bills of lading for <br /> 8) Violation 304: no spill control equipment was available onsite <br /> 9) Violation 404 and 605: one open 55-gallon hazardous waste container of saturated absorbent <br /> was observed at the time of inspection <br /> 10) Violation 605 and 701: two 55-gallon containers had the accumulation start date of May 15, 2012 <br /> and were stored onsite in excess of 180-days <br /> 1 <br />
