Grant St., Ste. 6 & Ste. 8 Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS 1,CLASS 11,or MINOR-Notice to Com I <br /> This is a routine Hazardous Waste (HW) inspection. #20 is a Repeat Class II violation, while #30 and <br /> #50 are Minor violations Notice to Comply: <br /> #20. (Repeat) Failed to completely label HW container. <br /> * HW containers must be labelled with the following informations at all times: 1) the words <br /> "Hazardous Waste", 2) Generator name and address, 3) Accumulation start date, 4) Content, 5) <br /> Physical state, 6) Hazardous property. <br /> -- Immediately label all HW containers as above and continue this practice. Violation has been <br /> corrected on site. <br /> #30. Observed three 55-gallon empty HW containers with no marking of the date they were emptied. <br /> * Empty HW or Hazardous Material (HM) containers greater than 5-gallons shall be marked with <br /> the date when they first became emptied, and managed within 1 year of that date. <br /> -- Immediately manage the above three empty HW containers by either reuse, recycle, or <br /> disposal. Begin marking all future empty HW or HM containers with the date when they first became <br /> empty, and manage them within one year of that date. Submit evidence of correction to this office by <br /> July 13, 2010. <br /> #50. Unable to determine the length of storage of one 55-gallon drum of used oil (full). The last <br /> disposal receipt for used oil the owner had on file was dated December 2009. <br /> * HW shall be stored on site no longer than 180 days. <br /> Immediately dispose the above 55-gallon drum of used oil and submit a copy of the disposal <br /> receipt to this office by July 13, 2010. Begin managing all future HW within 180 days of storage on site. <br /> NOTES <br /> - Explained to owner that drained used oil filters must be managed within 1 year of storage on site. <br /> - Explained to owner that the Emergency Coordinator information and Modified Contingency Plan he had <br /> on file must be posted by a phone.
