This information has not been <br />received resulting in a non-compliant status for this facility. <br />Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is <br />billed at the current hourly rate ($130). Failure to submit the completed information immediately <br />may result in a re -inspection, additional violations, and/or further legal action. <br />The missing information includes <br />Review the attached inspection report and complete the Return to Compliance Certification form <br />indicating how these violations were addressed of how you plan to address them.
