Failed to perform annual hazardous waste training for employees. The last documented <br /> "Hazardous Material Emergency Response, Spill Prevention, Mitigation and Contingency Plan"was <br /> performed 02/10/2010. The course topics were emergency response equipment use, maintenance and <br /> inspection; communication system use; emergency response to fires & explosions; ground water <br /> contamination; emergency shut down operations. Training shall be provided by a person trained in <br /> hazardous waste management procedures and shall include instructions which teat es facility <br /> personnel hazardous waste manaaemen procedures, including contingency plan implementation <br /> relevant to positions in which they are employed. At a minimum, training shall Include emergency <br /> procedures, emeMen_ cy equipmen , an emergency system, including w ere applicable: <br /> procedures for using, Inspecting, repairing, & replacing facility emergency and monitoring equipment <br /> 2. key parameters for automatic waste feed cut off systems <br /> 3. communications for alarm systems <br /> 4. response to fire or explosions <br /> 5. response to ground water contamination incidents <br /> 6. shutdown operations <br /> Records must include: <br /> w 1.job position title and name of person <br /> 2. written job description for above job position titles <br /> 3. written description of amount of training that will be provided to each individual <br /> 1 4. dates training conducted <br /> Each personnel shall be trained within 6 months of hire and be provided an annual refresher. <br /> Correct by providing hazardous waste training records to SJC EHD within 30 days (07/02/12). <br /> Hhr/ *L4O,Z0r,!e41 LJC-5I1� Rn 1daft <br /> Submit to SJC EHD within 30 days(07/02/12): * anlocyM\ t q3 )M ThV"u�L <br /> 1. one copy of the completed "Return to Compliance CertificationWi A W6JrctA,Z&iOM14-Cdtrl` <br /> 2. a written corrective action statement for each of the above violations/ —6wF(- Auno sna1" 0as� <br /> 3. supporting or requested documentation ie. training records l m woj�^ <br /> Notes: <br /> -Provided free training schedule. Recommended courses: Hazardous & Universal waste management, <br /> Aboveground Storage Tanks, Basic Auto Industry waste management <br /> - Fluorescent tubes maintenance done by outside contractor DM Lighting (Dave Miller) ph 601-1905 <br /> - Daily AST (waste coolant, waste oil, new oil) inspection log attached to waste oil tank. <br /> - In 2011 year, facility generated approx 2080 gallons (8.32Tons) of used antifreeze, 5191 gallons (20.76 <br /> Tons) of used oil, 0.7 Tons of manifested waste = 29.78 Tons of hazardous waste <br /> ALL END STAFF TIME ASSOCIATED YATH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br /> EHD Inspector.
