Class II Violation: #60,61,62 <br /> 60, 61, & 62. Manifest number 007419506JJK of waste paint shipped on 03/24/2011 was found without <br /> a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all <br /> manifests signed off by the destination facility on site for three years and have them readily available for <br /> review. If the generator did not receive a copy of the manifest with the handwritten signature of the <br /> owner or operator of the facility to which the generator's waste was submitted within 35 days of the date <br /> the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the <br /> owner or operator of the designated facility to determine the status of the waste. If the generator did not <br /> receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was <br /> accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible <br /> copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate <br /> the hazardous waste and the results of those efforts. Exception reports shall be submitted to: <br /> DTSC Report Repository, Generator Information Services Section <br /> P.O. Box 806, Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifest or prepare and submit an exception report to DTSC. <br /> Correct by submitting to the EHD a copy of the signed off copy of the manifest or a copy of the exception <br /> report submitted to DTSC. <br /> Submit to SJC EHD within 30 days (07/15/11): <br /> 1. one copy of the completed "Return to Compliance Certification" form <br /> 2. a written corrective action statement for the above violation <br /> 3. one copy of the manifest signed by the designated facility confirming waste was received <br /> Notes: - In 2010 year, facility generated approx 3435 gallons (13.74 Tons) of used oil, 2040 gallons <br /> (8.16 Tons) of used antifreeze, 3.11 Tons of waste paint liquid/solids and oily sludge = 35.01 Tons of <br /> haz waste <br /> - Potential Universal Waste found: unused microwave found in break area of Service Department and a <br /> small television monitor found near the office of the Body Shop. Universal waste can remain stored <br /> onsite for up to one year of initial date of accumulation. Item must be labeled as "Universal Waste - <br /> electronic devices". Ensure items are taken to an authorized hazardous waste disposal facility and retain <br /> disposal receipt onsite for a minimum of 3 years. <br /> - Fluorescent tubes maintenance performed by outside contractor who takes waste tubes. <br /> - Provided free training schedule. Recommended courses: Universal waste, Hazardous waste, Basic <br /> Auto Industry Hazardous Waste Management, Spill Prevention Control & Countermeasure <br /> -Waste oil & antifreeze tank assessment last done 77.
