This <br />information was required to be submitted by July 12, 2009. This information has not been received resulting in a <br />non-compliant status for this facility. <br />Failure to correct the violations will result in continuing/additional violations. <br />Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is billed at the <br />current hourly rate ($115).
