This Is a two page documettt. Please_carefully review the attached Special Waste Service Agreement as well as the terms and conditions on page 2. Please follow the Instructions below prior to scheduling and shipping material to the landfill: • Sign and date the Service Agreement in the space provided in the lower bottom left comer of the Service Agreement. • Initial and date the terms and conditions page in the lower left comer of the page. • Return the service agreement via email to the sender or Fax to: 209-466-1067. • Failure to return properly executed documents in a timely manner could delay our ability to provide you with excellent service. Schedulina Shipments & Non-Hazardous Waste Manifests: • Please schedule large quantity shipments 24 to 48 hours In advance. • Each load = be accompanied by a signed and dated Republic Services Non-Hazardous Waste Manifest. • Republic Services will pre-print and supply manifests to you. • If you have not received a manifest, please request manifests for your project by contacting Patricia Moreno at 800-204-4242 or via email: pmorenoQrepublicservices,com • A"wet"signature and shipment date is required by the authorized signatory on each manifest. • Any loads that arrive without a wet signature or the proper documentation will be rejected.
