There must be at least one <br /> emergency coordinator on site or on call to coordinate emergency response measures, and the following information <br /> must be posted by a phone: the name and phone number of the emergency coordinator; location of fire <br /> extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, unless <br /> the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required information by a <br /> phone. Approximately 15 intact lamps were observed <br /> in the dumpster area in danger of breakage. Universal waste lamps shall be stored in a manner that prevents <br /> breakage and release to the environment. All broken universal waste lamps and releases shall be immediately <br /> cleaned and containerized. Immediately store all universal waste lamps in a manner that prevents breakage and <br /> release to the environment. Containerize all broken and intact lamps and label with the words"universal waste <br /> lamps". Immediately dispose of this waste and submit a copy of the disposal receipt to the EHD. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by February 4, 2017 . <br /> An inspection report was provided to the facility operator on the day of inspection.
