This form may be obtained from any WCC health and safety <br /> officer. <br /> � o <br /> 502.3.2 RESPONSIIBLi: PARTY <br /> Reports of incidents occurring in the field shall be <br /> prepared by the site safety officer or, in the absence of the site <br /> safety officer, the supervising field engineer, witness,- cr <br /> injured/exposed Individual. ! <br /> 502.3.3 FILING <br /> A report must be submitted to the health and safety <br /> officer of the business unit to which the project manager belongs <br /> within 24 hours of each incident involving medical treatment. In <br /> turn, the health and safety officer must distribute copies of. the <br /> ro-port to the corporate health and safety administrator and the- <br /> corporate health and safety officer. When an :.njury or;; illness is <br /> " reported, the business unit health and safety officer must deliver ` 1 <br /> a copy of the report to the individual in charge of personnel <br /> affairs so that- a Worker's Compensation Insurance Report can be <br /> - filed if necessary.
