The emergency contact information listed Josuan Nieves as <br /> Coordinator and Nicholas Hernandez as Secondary. The emergency contact section did not list the State Om- <br /> of <br /> of Emergency Services. The contingency plan must include: <br /> 1. Description of actions facility personnel will take in response to fires, explosions, or any sudden or <br /> non-sudden release of hazardous waste to air, soil or surface water at the facility, <br /> 2. Description of arrangements made with local police departments, fire departments, hospitals, contractor- <br /> and State and local emergency response teams to coordinate emergency services; <br /> 3. List of names, addresses, and phone numbers(office/home/celq of all persons qualified to act as <br /> emergency coordinator. Where more than one person is listed, one shall be named as the primary emergency <br /> coordinator and all others in order in which they will assume responsibilities, <br /> 4. List all emergency equipment, which includes location, physical description, and brief outline of its <br /> capabilities; <br /> 5. Evacuation routes for facility personnel where there is a possibility of evacuation, <br /> 6. Current telephone number for State Office of Emergency Services. <br /> If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingent, <br /> plan, the owner or operator need only amend that plan to incorporate hazardous waste management provisions <br /> that are sufficient to comply with the requirements of this chapter. During the inspection, the emergency contact <br /> information was updated to include Josuan Nieves as Emergency Coordinator and Nicholas Hernandez as <br /> Secondary Emergency Coordinator. During the inspection, the emergency contact section was updated to <br /> include The State Office of Emergency Services and(916) 845-8510. Continue to amend the existing plan as <br /> changes occur to fulfill the requirements of this chapter. <br /> 111 Failed to maintain complete personnel training records. <br /> Employee training records for the management of hazardous waste conducted on2-16-16 were incomplete. The <br /> job titles and job descriptions were not documented. Employee training records shall include: <br /> 1. the job title for each position at the facility related to hazardous waste management, and the name of ea& <br /> employee filling each job, <br /> 2. written job description for each position listed above, <br /> 3. written description of the type and amount of both introductory and continuing training that will be given to <br /> each person filing positions listed above; <br /> 4. records that document that the training has been given to and completed by facility personnel. <br /> Training records on current employees shall be kept until closure of the facility. Training records on former <br /> employees shall be kept for at least three years from the date the employee last worked at the facility. During the <br /> inspection, the job titles and job descriptions were added to the2-16-16 training record. Continue to ensure that <br /> employee training is properly documented. <br /> 407. Contaminated container not marked with date emptied or managed within one year of being emptied. <br /> The empty 55 gallon drum of acetone was not marked with the date it was emptied. Containers larger than five <br /> gallons that previously held a hazardous material shall be marked with the date they were emptied and managed <br /> within one year of being emptied. During the inspection, the employee marked this drum with the accumulation <br /> start date. Continue to mark and manage all empty containers per this section. <br /> Page 6 of 6 <br />
