The generator and waste sections of the uniform manifest shall be completed and signed and dated by the generator. The transporter shall also sign and date the manifest. Ensure that all manifests are properly completed for any transport of a hazardous waste for off-site transfer, treatment, storage or disposal. Copies of uniform manifests for the disposal of hazardous waste for the last three years were not found on site.A single manifest was located on site with a date of June 15th, 2015. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies to the EHD. Manifest numbers 005232319JJK(06/15/15), 005232328JJK(6/18/15), and 005232324JJK(6/24/15) were found without a signed off copy from the destination facility. Hazardous waste generators shall r tain copies of all manifests signed off by the destination facility on site for three years and have them readily available fKreview. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or oper r o the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by t e i ilial transporter,the generator shall submit a legible copy of the missing manifest,with some indication that the ge erator has not received confirmation of delivery.
